Use case - Adding a Header section to a service request


When you select Header Section as the field type, the Required, Invoke Template, Display in Second Column, Validate and Hidden check boxes are disabled in the Prompt/Question Properties section. You can configure the Conditional and Scrollable properties only for Header Section Input fields.

You can select the type of separator from None, Line, and Bar that controls how the Header Section is displayed on the service request form.

  • None: By default, no separator is selected for the Header Section.
  • Line: Select this separator to display a line above the Header Section.
  • Bar: Select this separator to display the Header Section as a bar.

If you select another value as the Response Type, the Required, Invoke Template, and Hidden check boxes are enabled in the Prompt/Question Properties section. After creating a Header Section Input field and the other Input fields that appear under the Header Section Input field, you can select these Input fields and click the up arrow or the down arrow to modify the position of the Input fields in the display order of the service request.

For example, you create a Header Section Input field with the following Input/Prompt text:

Select the type of accessories that you want to request:

After creating this Input field, create check boxes for each accessory that is available in your stock, such as Monitor, Keyboard, Mouse, or Headphones.
